Amplar Health Home Hospital has expanded its Transition Care Service to Walkerville — providing another welcoming location for public hospital patients who no longer require acute care. Here, patients can rest and recover in comfort, while helping free up hospital beds for those who are acutely unwell. This expansion builds on our innovative model of care, enabling patients to move from metropolitan and peri-urban hospital wards into a supportive sub-acute setting designed for recovery and rehabilitation. No two days will be the same: you’ll care for patients recovering from surgery, illness, or injury, helping them regain strength and confidence, in this sub-acute nursing role.
