We’re home to an interdisciplinary team of experts – ranging from primary care providers to mental health specialists and gender-affirming surgeons. What you will do as a Social Worker: - Review Letters of Support and coordinate with perioperative coordinators to update clinicals for insurance coverage. - Support patients and their family through the emotional and educational process of accessing gender affirming surgical care. - Support patients’ navigation of resources and connection to healthcare services across the system and in the broader community, including but not limited to: identify and address gaps in care, support patient application for leave, write letters for legal name & gender change. - Assist patients with counseling related to resources and case management. - Assist patients in understanding the healthcare and managed care delivery system and act to resolve obstacles that prevent them from receiving adequate mental health car...
