Summary: The Advanced Practice Provider (APP) works independently, and in collaboration with the supervising physician to deliver excellent primary and chronic care management to a geriatric and palliative care population within long term care facility settings. The APP provides services within the APP scope of practice including the management of symptoms or disease processes related to acute and chronic conditions, assisting with communication and coordination of care, timely disease-specific education, as well as developing treatment plans that align with the patient’s goals throughout the disease process.
