Location: The Southern Territory Headquarters – Nashville, TN About this opportunity: Serves as lead worker to case management staff, serves as the first point of contact for questions about services; interviews accepts, and provides comprehensive, long-term, structured, complex case management services for an assigned caseload of clients participating in an emergency shelter and established transitional housing programs; understands the uniqueness of the client's history to determine most effective program plans; develops comprehensive program plan/goals and evaluates client's progress by conducting mentoring and counseling sessions with client and/or staff; serves as an advocate for clients to acquire services that will enable them to cope with their environment functionally; ensures constant compliance with funding requirements.
