VNA Care is seeking an LPN Clinical Admission Coordinator to join our Worcester team. Position Summary: Reviews and coordinates all needed clinical information prior to patient admission to ensure the perfect pathway for new patients and to ensure an exceptional customer service experience for the patient. Communicates with branches and teams regarding new patients and maintains positive, professional relationships with all internal and external customers. Rate of Pay: $25.00 - $38.00, rate of pay is based on experience. Schedule: On-Site: Monday, Wednesday 8am-4:30pm; VNA Care 67 Millbrook Street Worcester, MA 01606 Remote: Tuesday, Thursday, Friday 8am-4:30pm: Weekend and Holiday rotation: Current LPN License required.
