For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens, and currently offers 160-plus programs and services for children, youth, adults, seniors, and those struggling with mental illness. STATEMENT OF THE JOB: The Licensed Practical Nurse works as an integral member of the team providing services to senior residents who are aging in place or are clients in the Mental Health Clinics/PROS who are diagnosed with a behavioral health concern. The Licensed Practical Nurse is responsible for providing a range of wellness services to meet the behavioral health, medical, and care issues affecting those living independently within the Housing buildings and clients attending the Mental Health Clinics, Care Coordination or PROS programs. The Licensed Practical Nurse will be responsible for developing and implementing practices and protocols for clients ranging the entire life span. Residents in crisis or with behavioral health needs (such as hoarding, depression, anxiety, substance use disorders) will be reached out to, engaged and treated by the Licensed Practical Nurse.
