About the role: As a Senior Personal Carer, you'll conduct initial client intake and screening assessments, develop and review individualised care plans with a client-centred approach, while providing quality care based on clients' needs. Key responsibilities include: Conduct thorough initial screening assessments, developing care plans for personal self-care, general house cleaning, social support, respite, transport and shopping assistance. Manage client information in CRM & CMS ensuring all details and case notes are kept up to date.
