Department: 38947 Hugh Chatham Health at Home: NC - Home Health Status: Part time Benefits Eligible: No Hou rs Per Week: 0 Schedule Details/Additional Information: Various Pay Range: $30.40 - $45.60 What You'll Need: - North Carolina and American Speech / Language Association Certification - Must be a High School or GED Diploma Graduate. Masters in Communication Disorders What You'll Do: - Performs appropriate patient assessments resulting in an effective treatment plan. - Reassesses patients at appropriate intervals of time demonstrating the ability to observe treatment effect and modify the plan of care as indicated. - Involves the patient and /or family in the assessment process and the formation of the care plan. - Provides care in accordance with physician’s orders. - Demonstrates appropriate case management to ensure quality of care and best patient outcome. - Communicates appropriately to physicians, nurses, case managers, and other members of the health care team to ensure care is collaborative and appropriate. - Assesses and documents patient outcomes criteria at the initial and discharge evaluations.
