Clinical Social Worker: M-F (3 days a week) 8:30a-5p with some on call supporting Louisville and surrounding communities Clinical Social Worker Position Summary: As a member of the interdisciplinary team, provides social work services to patients and families as assigned. Services include: assessment, counseling, emotional support, assistance with financial and other social service needs, support and guidance regarding end of life decisions, legal affairs, and family assistance in planning for care giving.
