Salary Starting at $45.00 / hr Overview: RN UAS Assessment Nurse: Full-Time Special Project (3 Months) Elderwood IPA: Elderwood IPA is seeking experienced Registered Nurses (RNs) to support a special community health assessment initiative. In this role, the RN Assessment Nurse conducts New York State Uniform Assessment System (UAS) community health assessments for members in their homes or virtually. These assessments help determine functional status, care needs, and eligibility for long-term services. This position is ideal for RNs who enjoy assessment-focused work, independent field-based practice, and supporting complex populations through thoughtful evaluation and documentation. Unlike bedside roles, this position focuses on comprehensive patient assessments, clinical evaluation, and care coordination insight that directly supports care management teams and member service planning.
