Description NURSE PRACTITIONER SUMMARY : As part of a collaborative agreement with a physician as mandated by Ohio law and under the direction of the AxessPointe Community Health Center, Inc. (API) Medical Director, the NP/PA will provide quality primary medical and behavioral health services to adults, children and families in a holistic manner within the northeast Ohio area. SUPERVISOR: Medical Director or Designee: DUTIES AND RESPONSIBILITIES: - Examines, diagnoses, and treats illnesses and injuries of patients throughout the lifespan. - Provides well-child, well-woman and well-man health maintenance exams to new and established patients. - Orders appropriate screening and diagnostic lab and radiographic exams. - Interprets results of physical exams and screening/diagnostic tests to implement appropriate care plans for the patient. - Prescribes medicines, therapeutic agents and treatment modalities as required by patient needs and in accordance with licensed prescriptive authority. - Supports health promotion, health education, health empowerment and disease prevention interventions for all patients. - Provides age-appropriate care to all patients. - Promotes patient and family...
