Job DetailsJob Location: Community Support Services - Akron, OH 44311Position Type: Full Time (40 Hours)Salary Range: $23.54 - $27.90 HourlyThe Opportunity: We are seeking to hire an LPN – Community to join one of our regional treatment teams. This individual is an integral part of the treatment team, works closely with the behavioral health provider, primary care providers, behavioral health team members, and other community agencies. The LPN assists the RN Care Coordinator with coordination of multiple health care services and consults, medication management, ongoing evaluation of client progress, re-enforce education, and provide feedback to the entire treatment team. What You’ll Do: Assist the RNCC with identifying those individuals with identifiable risk factors (at risk in levels or blood pressure and weight/BMI risk level). Act as liaison between public and private health facilities and service agencies to carry out referral instructions. Participate in an interdisciplinary treatment team, cooperate with and utilize the resources of other community agencies as described above, including other CSS programs, in order to achieve treatment goals of individuals served.
