Description Hospice Social Worker – Make a Difference Every Day Join our team as a Hospice Social Worker and provide compassionate support to patients and their families during life’s most challenging moments. You’ll guide patients through admissions, care planning, and discharge, provide emotional and psychosocial support, advocate for their rights, and connect them with community resources. Be part of a dedicated, interdisciplinary team and help improve quality of life while making a meaningful impact every day. Job Status: Full-time (0.8 FTE, 32 hours per week) Salary: $27.69 to $34.35/hourly, based on experience. Higher pay may be considered depending on qualifications and experience. Schedule: M–F, 8:00 AM to 4:30 PM. However, flexibility is required, as hours may extend beyond the regular schedule based on patient and family needs.
