Department: 70518 Atrium Health Mobile Medicine - Care Coordination Center Status: Full time Benefits Eligible: Yes Hou rs Per Week: 36 Schedule Details/Additional Information: 7a-7p Pay Range: $38.20 - $57.30 Major Responsibilities: Patient Transfer Coordination: Facilitate safe and timely transfers of patients with emergent , urgent , or medically complex conditions; collaborate with physicians, transport staff, and other healthcare providers to ensure appropriate patient placement. Clinical Assessment: Perform triage assessments to determine the appropriate level of care and subspecialty needed for transferring patients; utilize clinical knowledge to gather and review patient information, ensuring all necessary data is collected for transfer decisions. Communication : Utilizing closed -loop communication a ct as a liaison between referring and receiving facilities, facilitating verbal communication and documentation of transfer requests; coordinate with multiple clinicians to discuss patient cases and determine the best course of action; provide exceptional customer service to patients, families, and external healthcare providers.
