The Glenview at Pelican Bay is a premier continuing care retirement community located in beautiful, sunny Naples, Florida. We are currently considering candidates for a hospitality focused Home Health Liaison. Candidates must have a desire to join a great team dedicated to providing outstanding and superior hospitality. We currently have the following shifts available: part time, 20 hours per week. The Home Health Liaison at The Glenview at Pelican Bay promotes and represents our home health agency, GlenCare to physicians, hospitals, senior living communities, rehabilitation centers, and other referral sources while building relationships and increasing patient referrals. The Home Health Liaison is responsible for the following services including but not limited to: - Develop and maintain relationships with physicians, hospitals, discharge planners, and other referral sources. - Educate healthcare professionals and the community about the agency's services - Generate new referrals and meet monthly growth targets. - Collaborate with clinical and administrative staff to ensure smooth patient intake. - Track marketing activities, referral patterns, and outcomes. - Represent the agency...
