Job Summary Provides care and support in residential setting to the people we serve by assisting in the development and implementation of individual Support Plans, including spiritual life supports, assisting people in achieving their personal goals and desires, providing interaction and choices for activities that support a meaningful day, supporting people to be independent in activities of daily living and personal care as needed Essential Duties 85% Direct Care & Support ◦ Provides personal care such as bathing, dressing, bathroom assistance or housekeeping. ◦ Prepare and serve meals in accordance with individual diet plans. ◦ Administers medication, prescriptions as documented under the direction (supervision) of a nurse. ◦ Schedule as requested and take individuals to medical appointments, outings, or errands. ◦ Support engaging activities to maximize and enhance the quality of daily living and support individuals’ personal goals and quality of life. 15% Other Program Support The above reflects the general details necessary to describe the principle and essential functions of the position and shall not be construed as the only duties that may be assigned for the position.
