Employment Type: Full time Shift: Day Shift Description: RN Care Manager Full time: Hybrid/Remote options once trained: Local candidates only: This position is an ambulatory care registered nurse who will work focusing on our Trinity Health medical group patients. This position will focus on patients with multiple chronic diseases, CHF, COPD. This position will focus on disease education, identifying and mitigating social influencers of health, coordinating needed services and collaborating with the provider offices to ensure integrated care through the healthcare continuum and improved health outcomes of our patients. What you will need: Associates Degree required, BSN preferred. Previous experience required in care management at least one of the following settings: disease management, home health care, hospital or physician office setting. Certification in Case Management (CCM) preferred. Experience Requirements: 5 years of RN experience. Prior electronic medical record (EMR) experience required.
