Reablement Assistant Practitioner | 10551 | Permanent contract | 37 hours per week | £32,061 to £33,699 per annum | (Grade G) | Greyfriars House, Great Yarmouth Would you like to work in the community as part of our well-trained team of Occupational Therapists and support workers, creating person-centred personal plans helping people set and achieve their goals following an illness? Then this role is for you. We have opportunities available to visit people in their own homes across East Norfolk. Norfolk First Support delivers services to adults across Norfolk in their own homes for up to 6 weeks, assisting them to regain skills they may have lost following an illness or hospital admission, enabling them to regain their independence as much as possible while also reviewing any ongoing support needed. In this role, you will be part of a multidisciplinary assessment team, working alongside other professionals in Social Care supporting people who may have been discharged from hospital or where they have a change of circumstances and require support at home.
