Salary Rate $61,901.69-$79,848.88 Essential Functions: - Determines eligibility, acceptance, and placement for individual(s) with substance use issues by performing intake and assessments, completing an ASAM criteria assessment. - Coordinates with internal team to identify recovery plan goals and coordinates closely with internal medical care coordinator to ensure all domains of client recovery are incorporated into the treatment planning. - Develops treatment plans that are driven by the client and encompass change management practices including the use of motivational interviewing. Assists clients in creating treatment plans, monitors treatment plan progress, and maintains progress notes. - Provides case management assessment of individual needs, makes referrals for services when appropriate, completes chart forms and documentation. - Provides counseling and alcohol/drug education to program participants, including individual counseling, group counseling, family counseling/family unity meetings, and relapse prevention/crisis intervention. - Consults with community resources and outside agencies such as community corrections, courts, probation officers, Social Security Administrat...
