FTE: 1 Shift: Day Schedule: Clinic hours Position Summary: As an Ambulatory RN Care Manager you’ll be responsible for exemplary cross-continuum care coordination, orchestrating care across the health care system and identifying opportunities to improve patient care, services and overall population health. You’ll employ the nursing process in the delivery of patient-centered nursing care and work collaboratively with multiple disciplines and resources facilitating wellness, and transitions of care for patients in the ambulatory environment.
