S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. The Hospice Liaison is responsible for the development of the hospice program through processing consents and managing the pipeline of potential patients. Licensed Practical Nurse is preferred for this role. Position Specific Responsibilities: - • Provides accurate information regarding hospice services in response to inquiries by healthcare providers and general public. - Processes all consents as assigned, as is measured on conversions of referrals to admissions. - Conducts informational visits. - Manages the pipeline of potential patients on a daily basis, including following up with Hospice Care Consultants, growth leaders, referral sources, and patients and their families. - Participates in community and organizational programs as reques...
