Job description

Team Collaboration: - Collaborate with Case Managers, Recreation Coordinator, and Shift Coordinator to provide direction for resident activities - Collaborate with Case Managers to interpret goals from Individual Service Plans for use with Daily Documentation - Attend Behavior Intervention Planning (BIP) meetings, MDT meetings, and any mandatory training, as directed - Assist in therapeutic interventions of residents - Welcome & mentor new staff, serve as a role model - Assist in the development and coordination of recreational activities - Serve on various committees for Performance Improvement & Individual Career Path Safety & Supervision: - Monitor the whereabouts of residents at all times according to the agency’s policy and procedures - Maintain knowledge of all resident information sheets and resident safety plans including 1:1 - Provide Adult/Child CPR/First-Aid during emergency situations - Provide appropriate SCM interventions according to their certification, when a resident is harming himself, others or when imminent harm is present - Ensure proper resident hygiene of assigned residents Data Collection & Analysis: - Track and calculate Daily Documentation scores and upda...
