Job description

Work Shift: Day - 8 Hour (United States of America)FT, 11a-7p Position Summary: Primarily responsible for coordinating the continuum of care for all patients referred to the ED. Will facilitate radio and telephone communication for incoming ED patients. Obtain and update all documentation in the electronic medical record to include but not limited to patient history, diagnosis, transfer facility, call back numbers, etc… Consult with ED physician and charge nurse to evaluate severity of illness and intensity of services to be provided and to determine appropriateness of ED acceptance of transfers. Acts as a liaison with ED Physician and consulting physicians. Facilitate and/or assist with admissions by collaborating with transfer facility, charge nurse, house supervisor or bed coordinator. Will obtain verbal (telephone) orders as necessary. Ensures a positive and seamless interaction with the ED and other disciplines/units/facilities to ensure optimum patient care. Follow up on lab and radiology results and consults with physician or designee in order provide full follow up for proper treatment (prescriptions and follow up calls).
