JOB SUMMARY: The discharge planning assistant is responsible for providing accurate and timely administrative and technical support to the discharge planning and utilization management teams, helping ensure a positive patient experience, effective coordination of care, utilization review processes, and compliance with both hospital policy and CMS guidelines.
