RN Hospice (Field) - PRN Job Description: Registered Nurse is responsible for identifying patient/family needs, and for providing nursing and supportive care for terminally ill patients and their families/caregivers. Manages a caseload of patients/caregivers as assigned. Responsibilities: Assessment- Collects pertinent data and information relative to the patient's health or the situation. Recognizes normal and abnormal findings when gathering data. , Attends to patients and families at the time of patient’s death., Coordination of Care- Plans and evaluates care in collaboration with appropriate disciplines for assigned patients., Diagnosis - Analyzes assessment data to determine actual or potential diagnosis, problems, and issues., Evaluation- Evaluates progress toward attainment of goals and outcomes.
