Chaplain - Full Time - Day Shift - Southampton Hospital POSITION SUMMARY: Provides spiritual care to meet the spiritual, religious, emotional and psychological needs of selected Palliative Care patients and families of all faiths and backgrounds. RESPONSIBILITIES: - Assesses and documents the spiritual needs of selected Palliative Care patients and their families and documents in the chaplaincy needs assessment. - Identifies caregiver burden for spiritual and emotional needs. - Performs spiritual rounding on selected Palliative Care patients, providing spiritual counseling to patients and families as needed. - Attends team meetings and participates in interdisciplinary review of the plan of care for patients and their families. - Serves as a liaison to the patient/family/ community clergy in response to the needs, wishes and beliefs of the patient. - Provides education and support about Palliative Care, hospice, spiritual care, death and dying and bereavement. - Supports spiritual/emotional needs of staff during crises as part of the Palliative Care team. - Potential for community home visits to provide spiritual care, meeting the spiritual, religious, emotional and psychological n...
