FTE: _0, Shift: Day_, Schedule: _Per Diem Position Summary: To provide a single point of focus for rehabilitation patients as they move throughout the continuum of care to assure smooth, appropriate, coordinated, timely and resource sensitive transitions and assuring those services provided are in the best interest of the patient and family. This position operates independently, working cooperatively and collaboratively with patient, families’ health care providers, payors, and management. Work situations require substantial clinical competency and specialized knowledge, judgment, and skill.
