Job Summary: Provides psychosocial evaluations, psychotherapeutic interventions, case management and discharge planning services for patients and/or their families. Provides and acts as a liaison for physician, patient and/or family between the hospital, community agencies and facilities. Performs intake assessment/evaluation of family dynamics and treatment needs, including creation of treatment plan. Prepares case history of patient's personal, social, and emotional development. Assists patients to evaluate both their strengths and symptoms, and facilitate patients setting their own goals, and plan for appropriate services. Provides therapeutic intervention with patient family including individual, marital, parental, family and group therapies. Interprets nature of illness and prescribed course of treatment to patient and families. Communicates family issues to other staff members.
