WHAT YOU WILL DO: The Safe at Home (SAH) Case Manager is a non-exempt position responsible for leading the facilitation of team services in the Safe at Home program model. This position works in collaboration with the Family Advocate and Counselor to build the natural support of the family, including providing linkage to formal (community providers) and informal (family, friends, neighbors, church, school, etc.) resources to clients involved with Florida Child Protective Services to ensure child safety needs are met in conjunction with the provision of family strengthening interventions. - Conduct home visits with parents, caretakers, at-risk children, their families, and extended support networks - Provide information, referral, and follow-up for appropriate ancillary community counseling, psychiatric services, financial, employment, parenting, housing, and other services to meet family needs - Create individualized plans and needs assessments, and c...
