Community Based Case Manager

Job description

ESSENTIAL FUNCTIONS: - Provide goal-directed, strength-based, and recovery-oriented services in community settings (home, school, etc.). - Collaborate with clients, families, and treatment teams to develop and monitor individualized treatment and crisis plans. - Deliver supportive counseling, emotional and behavioral interventions, skill-building, and crisis response. - Coordinate services with schools, healthcare providers, social services, and other community resources. - Assist with benefit applications, transportation arrangements, and accessing essential services. - Conduct strengths and needs assessments, complete and update treatment plans and clinical documentation. - Serve as an advocate for clients and families and act as a liaison across systems of care. - Participate in internal meetings and external networking to enhance service access. - Maintain confidentiality, promote cultural competence, and adhere to trauma-informed practices. - Actively support and promote the agency’s mission and strengths-based philosophy. - Complete required background checks every two years. - Complete required training. - Work as part of an integrated treatment team collaborating with the i...
