This role ensures timely, compliant, and clinically appropriate admissions while supporting strong communication between referral sources, clinical teams, and leadership. The ideal candidate is a Registered Nurse with hospice or home health experience who brings strong clinical judgment, operational leadership, and a passion for improving patient access to hospice care. This position plays a key role in ensuring referrals are processed efficiently, eligibility is appropriately assessed, and patients receive the care they need as quickly as possible. Key Responsibilities: Clinical Review & Eligibility: Review incoming hospice referrals to determine eligibility based on Medicare and regulatory guidelines Collaborate with the Medical Director and clinical leadership on complex eligibility decisions Ensure documentation supports terminal diagnosis and regulatory requirements Identify and escalate potential compliance risks Intake Operations: Oversee the daily operations of the centralized int...
