Job DetailsJob Location: AmeriCare Home Health Care - Naples , FL 34105Salary Range: $24.00 - $42.00 Hourly*This position comes with a $5,000 OR $10,000 Sign on Bonus based on licensure for qualified candidates! Required Schedule: Monday - Friday 8:30am-5:00pm Job Summary: The RN utilizes the nursing process of assessment, planning, intervention, implementation and evaluation. The RN is skilled at assessment and management of pain and other complex symptoms, including education and support to patient and family. The mission of the home health team is to meet the unique needs of clients and families with services tailored to assist them in achieving optimal level of function. The Patient Access Nurse coordinates and completes the referral and admission process to ensure patients smoothly transition to home care services. Job Duties: Essential Job Functions: Routinely meets individual goals, and assists the team in meeting departmental goals. Meets all requirements of the Home Health Registered Nurse.
