Employment Type: Full time Shift: Day Shift Description: Full-Time Occupational Therapist Liaison If you are an Occupational Therapist looking for a different type of Occupational Therapy position, this opportunity located at St. Peter's Hospital is for you. This position is a member of the Complex Care interdisciplinary team and is responsible for ensuring patients transition to the next best site of care. What you will do: - Directly co-treat patients in collaboration with the primary therapists, providing supplemental therapies. - Utilize your clinical expertise in occupational therapy to provide education and resources to ensure patients remain in the least restrictive environment when transitioning from acute care. - Serve acute care & complex care team, ensuring the no place like home approach is utilized in all appropriate scenarios. - Ensure timely progression to the next site of care by implementing appropriate occupational therapy treatment to vulnerable in need. - Responsible for educating case/care management teams, social work/utilization management services as well as the greater hospital system on functional status, DME options and alternatives to inpatient rehabilit...
