Job Description​ Join Saint Luke's Home Health and Hospice in a NEW ROLE!! We are seeking an RN to join us as a Clinical Intake Coordinator! FT: M-F 9a-5:30p: Hybrid role!! Remote 2-5 days per week! Alternating weekday and weekend shifts every 4-6 weeks Holiday rotation: RN MO and KS required: Home Health or Hospice experience preferred Experience with referral acceptance and appeals preferred The Opportunity: The Clinical Intake Coordinator facilitates the home health and hospice referral process by receiving referrals from multiple referral sources within and without the St. Luke's Health System, performs initial evaluation of referral for home health appropriateness under Medicare, Medicaid, and commercial insurance regulations, determines need for additional information from referral sources, communicates agency acceptance or decline of referral to varied referral sources, and processes accepted referrals. The Work: Facilitates the home health and hospice referrals. Provides specialized support for the Patient Accounting and Finance departments.
