Social Worker | Blue Care | Cairns Community Care Employment Term: Permanent Full-Time: Location: Cairns and surrounding locations: Remuneration: $42.15 - $59.04 p/hr (dependent on experience) + 12% Super + Not for Profit Salary Packaging + Penalties & Allowances 5 weeks annual leave Looking to relocate? Our dedicated teams are located across Queensland to provide the best inhome support to our clients. As a Social Worker, you will support our clients living independently in the community by providing psychosocial assessment, care coordination, advocacy, counselling, and support services that improves their quality of life and well-being. Key responsibilities include but not limited to: Conduct comprehensive psychosocial and risk management for clients in the community Provide counselling, emotional support, crisis intervention, and advocacy Develop and review individualised care plan plans Work collaboratively with a multidisciplinary team including personal care staff, nursing and allied health Facilitate referrals to community support, hea...
