Description Nashua, NH: About the Role: Greater Nashua Mental Health is seeking a Mental Health Court Case Manager to provide evidence-based case management services to justice-involved adults with serious mental health and substance use disorders. This role supports participants in meeting court requirements, accessing treatment, and building long-term stability and recovery while serving as a liaison between the court, legal partners, and treatment providers. Key Responsibilities: - Complete intake and ongoing case management assessments and develop individualized service plans. - Coordinate and monitor participant engagement in appropriate levels of care. - Track and report participant progress and compliance with Mental Health Court requirements. - Address key recovery needs including housing, transportation, employment, and community supports. - Connect participants to medical, educational, social, and community resources. - Provide direct and indirect case management services. - Facilitate individual and group Enhanced Illness Management and Recovery (EIMR) services, including leading at least one weekly group. - Attend weekly supervision, Mental Health Court staff meetings,...
