Woodfords Family Services is seeking a dedicated, relationship-driven professional to join our team in Manchester, ME as a Shared Living Coordinator! This full-time (40 hours per week) position offers a competitive compensation and benefits package, with eligibility for a hybrid schedule (one remote day per week) following the introductory period. Position Summary: Woodfords' Shared Living Program is designed to provide family, home-based services to individual's ages 18+ with intellectual and/or developmental disabilities. An alternative to group home living, adults are matched with provider families who deliver a stable support system and natural family environment that fosters relationship building and community inclusion. The Shared Living Coordinator provides oversight support services to achieve program needs and advocate for consumers' freedom of choice. Additionally, the Shared Living Coordinator will serve as a liaison between providers, family/guardians, and other service providers to help establish and maintain the HCBS Global Settings Rule and monitor progress toward consumer goals and objectives.
