Job description

Minimum Qualifications Bachelors degree in Human Service Field, Business or related field required. Masters degree preferred. One to two years of job development, Talent Acquisition, training specialist or case management experience preferred. Excellent written and verbal communication skills. Responsibility: · Participates as a contributing full member of the multidisciplinary treatment team. Works in collaboration with Team Leader/LPHA on clinical needs of assigned caseload. · Provides PROS members with employment counseling, advocating services, benefit counseling (Social Services, Social Security, Medicaid). · Performs job development and career development activities based on the needs of PROS members. Completes quarterly State required reports. · Develop/facilitate class curriculum for skill building classes. · Provide CRS, IR, Clinical Symptom Monitoring and ORS interventions as outlined in PROS participant IRP. Symptom monitoring services will be reviewed and co-signed by MD/RN.
