Description POSITION DESCRIPTION: Job Title: Registered Nurse – Home Health, FT; M-F, 8am-4:30pm Supervised by : Director of Home Health Job Summary: Provides nursing care to patients in their homes, ensuring that they receive the highest level of clinical care and support. The RN will assess patient needs, develop care plans, administer treatments, and educate patients and their families about managing health conditions at home. This role requires strong clinical skills, the ability to work independently, and excellent communication with both patients and the healthcare team. Demonstrates Competency in the Following Areas: - Ability to perform a head-to-toe assessment on all patients and reassessments as per policy. This includes: pediatric, geriatric and the general patient population. - Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques.
