Registered Nurse - Hospice - Overnights

Job description

We are seeking a Registered Nurse to join our Hospice RN team. The ideal candidate will provide high-quality compassionate end-of-life care. Setting : Hospice care facility. Schedule : Overnights 7pm-7am, occasional weekends. Essential Functions: - Delivery of Patient Care - Assumes responsibility for the coordination and delivery of patient/caregiver care. - Conducts nursing assessments to assist in the overall management of patient/caregiver needs. - When appropriate, develops, implements, evaluates, and updates the plan of care to ensure that care is uninterrupted and that goals are achieved. - Facilitates patient and caregiver independence to the extent possible. - When providing hospice care, appropriately utilizes VIA Health Partners patient related protocols, standing orders and guidelines. - Assumes responsibility for conducting effective patient and caregiver education. - Encourages and allows time for questions and consultation. - Provides education to patients and families as needed. - Communicates effectively with members of the IDG, patients, caregivers, and other health professionals to ensure continuity of care. - Maintains open lines of communication with supervisor...
