Winchester Gardens is seeking a Director of Social Services to join our team! Winchester Gardens is a Springpoint Senior Living community located in Maplewood, NJ. We are seeking a Full-Time Director of Social Services to join our team. At Springpoint, our Director of Social Services primary purpose is to provide medically-related social services to attain or maintain the highest practicable physical, mental and psychosocial wellbeing of each resident. “Medically-related social services” means services provided to assist residents in maintaining their ability to manage their everyday physical, mental and psychosocial needs. Responsibilities: - Develop and implement case management services to all residents - Make arrangements for obtaining needed adaptive equipment, clothing and personal items - Maintain contact with family (with resident’s permission) to report on changes in health, current goals, discharge planning and encouragement to participate in care planning - Maintain census - Maintain progress notes under federal and state guidelines for Long Term Care - Assist staff to inform residents and those they designate about the residents’ health status and health care choices an...
