Become a part of our caring community Humana Gold Plus Integrated is seeking Long-Term Services and Support (LTSS) Care Coordinators (Care Coach 1) in the state of Illinois to assess and evaluate members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families towards and facilitate interaction with resources appropriate for the care and wellbeing of members. The LTSS Care Coordinator (Care Coach 1) employs a variety of strategies, approaches, and techniques to manage a member's health issues. The LTSS Care Coordinator (Care Coach 1) understands own work area professional concepts/standards, regulations, strategies, and operating standards. Work is managed and often guided by precedent and/or documented procedures/regulations/professional standards with some interpretation.
