RN / LPN Transitional Care Manager, Home Health Visits

BAYADA Home Health Care is in search of a nurse to serve as a Transitional Care Manager for our Home Health Visits service line. The Transitional Care Manager (TCM) will build and strengthen relationships between referral sources and the company, while educating about company services and the benefits of home health care. He/she will use clinical knowledge to educate patients about home health service eligibility and expectations, and will manage referral intake and coordinate the initial transition to home care. This is an in-person position, that will require daily travel to area hospitals. Qualifications for a TCM: - Current license in good standing as a RN or LPN in the state of practice - Demonstrated record of strong interpersonal skills. - Demonstrated knowledge of the Medicare COPs and other program requirements, as applicable. - At least one (1) year experience in health care, preferably home health care. - Previous discharge planning experience preferred.
