Park Nicollet is looking to hire a Nurse Navigator to join our Breast Health team! Position Summary: The Nurse Navigator in Breast Health is a nursing practice role serving patients with breast cancers and their families throughout the Park Nicollet integrated health system. This role identifies patient needs from diagnosis through the care continuum, coordinates care and provides consultation and collaboration with interdisciplinary teams (including surgery, oncology, radiation therapy, radiology, and pathology). Responsibilities include assessment, intervention, ongoing evaluation of physical, psychosocial, and financial needs, patient/family education, and resource identification.
