Description: The School of Nursing seeks a Dementia Care Assistant II to work with our Integrated Memory Care Unit. KEY RESPONSIBILITIES: - Implements recommended strategies and activities outlined by the APP's plan of care within the client's home/assisted living community. - Documents activities completed with the client during 1:1 encounters in a way that supports the time spent; Completes all necessary paperwork and computer documentation in EPIC. - Gathers information from senior living community staff regarding any changes with the participant. - Communicates any changes with participant to Community Programs clinical staff. - Attends and actively participates in interdisciplinary team meetings in order to obtain and share information in service of providing holistic primary care. - Respects the confidentiality of all client information and conforms to clinic/departmental policies on the handli...
