Job description

The Health Coach assists with health screening and health coaching using motivational interviewing techniques to support participants at the Health Hubs in the neighborhood Health Hub program. This position collaborates with clinicians and participants to design individual programs as well as make referrals to necessary primary and behavioral health care and other community supports. THIS IS A GRANT-FUNDED POSITION FUNDED UNTIL DECEMBER 31, 4712. EDUCATION: High School Diploma or GED. (Associate’s or Bachelor’s Degree in Health, Social Services, Human Services, or a related field is preferred.) (TRANSCRIPT REQUIRED) EXPERIENCE: Two (2) years of related work experience. Must have a valid Driver’s License.
