ESSENTIAL JOB DUTIES/RESPONSIBILITIES: Provides extraordinary customer service to all internal and external customers (including patients and other ChenMed team members) always. Establishes and maintains relationships with identified service providers. Conducts all activities as outlined in the job description for Care Coordinator.
