CLINICAL CARE: Assessment - Conducts an effective clinical assessment for assigned patients, utilizing multiple sources/methods for data collection, and interprets data to create an individual plan of care. Treatment Planning - Develops intervention/treatment plans, including specific goals and a variety of direct and consultative intervention approaches, while maximizing available resources within both hospital and community networks. Treatment Implementation - Partners with the patient, family and health care team members to define and implement physical therapy interventions that support identified needs relative to prevention of delays, promotion of independence, remediation for loss of function and compensation for deficits. Provides education to patient and families, monitors response to treatment plan and revises accordingly. Seeks assistance as needed. Population Specific – Demonstrates knowledge of the differences and needs of patient specific populations from infancy through early adulthood and applies them to practice.
