The aim of this role is to reduce unnecessary social hospital admissions/ to support carer breakdown and to promote independence, by assessing people in their own homes and assisting people to maximise independent living. Our focus includes community support before hospital transfer and assistance in Accident and Emergency departments. As a Level 2 Social Worker, you will assist the manager in delivering a holistic service, ensuring the safety of vulnerable adults, and facilitating prompt assistance for adults and their caregivers during crises such as falls or caregiver emergencies.
