Job description

We're looking for a Licensed Practical Nurse (LPN) focused on remote patient monitoring and hospital avoidance. This role requires an organized professional to monitor patient data, conduct timely follow-ups for concerning symptoms, coordinate care, and actively engage patients to ensure adherence to their care plans and device usage. Job Responsibilities: - Proactively manage patient health by monitoring remote patient monitoring (RPM) dashboards and responding to vital sign alerts (e.g., BP, glucose, weight) - Conduct regular outreach calls to patients to assess symptoms and encourage consistent daily device usage - Coordinate timely follow-up care with Nurse Practitioners (NPs), Primary Care Providers (PCPs), or patient caregivers - Maintain accurate documentation of all patient encounters within the EMR and internal tracking systems - Ensure patient compliance and smooth operation by troubleshooting remote monitoring device issues - Act as the primary clinical point of contact for an assigned panel of remote care patients - Assist with patient enrollment and onboarding activities as needed to support growth. - Collaborate daily with the clinical team (NPs), intake staff, and su...
